Our Policy

Raquel K Perlis Physical Therapy is an out-of-network, fee-for-service practice. This arrangement gives the therapist the freedom and flexibility to treat you for a full 50 to 60 minutes, without any time or treatment restrictions imposed by insurance companies.

We do not accept Medicare or Medicaid at this time.

Most insurance plans with out-of-network benefits will reimburse members for their out-of-pocket payments to us. Be sure to check with your insurance provider. We're happy to provide receipts and statements noting procedure codes, which you can submit for reimbursement to your insurance company, and you can use your health care savings account (HSA), or flexible spending account (FSA) to pay for services.

Please pay in full at the time of service. We accept cash, personal checks and most credit cards.

To avoid a $85 fee, please call us at least 48 business hours before your appointment if you need to cancel or reschedule. Your courteous advance notice allows another patient to receive treatment.
